I screwed up today. I turn my mirror body's in reverse so I can turn from the middle to the outboard side. This lets me turn down hill with the grain and I have the tool handle against my body for better control. I usually snug the chuck up against the spindle and then use the chuck key to tighten it further. This has worked quite well over the years. Today I forgot to tighten it. It came unscrewed and forced the wood against the tool rest burning the wood.
I was able to save the mirror. I turned away the burned marks and then added beads to the backside.

Looks like you fixed the screw up very well. Do you cut your own mirror or have it done? Maybe it's part of a kit?

Russ I buy my mirrors in quantity from a company called TRipar. They have a $120 minimum order but I go through that many in a year so it's not too bad. I use 4" mirrors mostlly but have used 2" and 3" . A friend who just quit making the smaller mirrors gave me about 50 blanks so I've got to order some of that size.
